I know primals drop at approximately a 1/400 rate, which does NOT mean getting 400 legendaries guarantees a primal ancient. Statistical anomalies exist, and sometimes one might go through a significant drought of not finding any. All that being said, there IS a switch in the game that controls whether primals are dropped in general.

They will drop randomly from basically anywhere. Best bet is to farm bounties and reforge your legendaries in the cube. Get a character that can run GR80-90 in sub two minutes. Run a billion of those. If you end up with excessive forgotten souls run some bounties to reforge an item you want a primal up.
